What is DICOM?

DICOM (Digital Imaging and Communications in Medicine) is the international standard for the acquisition, storage, distribution, and display of medical images. Published by ACR and NEMA, the current specification (PS3.x) spans more than 20 parts covering everything from the network protocol to file formats.

Every DICOM image is an object that contains both the image pixels and all the patient and study metadata (name, date, modality, acquisition parameters) in a standardised data structure. This means a CT acquired on a Siemens scanner can be displayed immediately on a Philips workstation without any conversion.

A PACS like Medisfera acts simultaneously as an SCP (Service Class Provider) to receive and serve images, and as an SCU (Service Class User) to query and retrieve images from external systems.

C-STORE SCP

Image storage

Receives DICOM images from modalities via C-STORE. SOP class validation, automatic indexing, Transfer Syntax support including JPEG-LS, JPEG 2000, and Deflated Explicit.

C-FIND SCP/SCU

Search and query

Query studies, series, and instances. All query levels supported (Patient/Study/Series/Instance). Filter by date, modality, AE Title, and custom attributes.

C-MOVE SCP/SCU

Node-to-node transfer

Retrieval and forwarding of complete or individual studies to remote DICOM nodes. Forward queue with exponential backoff retries and dead-letter queue.

MWL SCP

Modality Worklist

Modality Worklist Server providing patient and procedure data to imaging modalities. Real-time synchronisation with RIS/HIS via HL7 SIU/ORM.

MPPS SCP

Procedure status

Receives procedure start and completion notifications (N-CREATE/N-SET). Automatically updates worklist status and the scheduling system.

REST API for images

QIDO-RS (search), WADO-RS (retrieval/rendering), STOW-RS (storage). Access from any web client without proprietary software. Foundation of the OHIF viewer.
